Indoor Rules for Fogerty

If you are under 14 years old you will need to fill out a Form for the Department of Labor.  See instructions below.

If you want to be included on the next indoor schedule, April 6 to April 27 you will need to get me all your information by March 25.

Indoor Information sheet, W-9 (1 per year), Waiver, Availability form, 

You must return the Availability, W-9 (1 per year), proof of age (if under 18), and Waiver before

I can schedule you for any games.

If you still need to attend a Summer Clinic you can visit the MN-SRA web page.

If you can't read any of the links here you may need to update

your Acrobat Reader.    Click the icon to download, 

save it to your desktop so that you can find it after it is done downloading.

After you download Acrobat Reader, you will need to install it on your computer.

ALL REFEREES UNDER 14 YEARS OLD MUST COMPLETE A FORM FOR THE
